最近氣溫漸昇，雖未入夏，卻已傳出多起食物中毒事件，尤其近日爆發之肉毒桿菌毒素中毒致死案，更是令人憂心。中縣衛局呼籲，於氣溫屢創新高之時，須更加注意飲食衛生安全。 　　 肉毒桿菌（Clostridium botulinum）是一種極厭氧的細菌，環境惡劣時會形成耐受性較高的孢子，其毒素產生有特定條件：無氧、高水活性、低酸性(pH>4.6)及適當的溫度等。造成中毒之發作時間及嚴重程度，與毒素含量多寡有關。潛伏期越短，症狀通常越嚴重，多數於攝食含有此種毒素食品約18-36小時後開始出現症狀，也可能早於6小時、或晚至10天才出現。主要症狀為神經麻痺，包括視力障礙、瞳孔放大、眼皮下垂等眼部症狀，及言語障礙、吞嚥困難、唾液分泌障礙、口乾等。如未即時適當治療，漸會有弛緩性麻痺現象，也可能嘔吐、便秘或下痢等，嚴重者會因呼吸肌肉麻痺導致窒息而死亡。 　　 儲存不當之真空包裝及鹽漬類產品，是較可能引發肉毒桿菌中毒的食品，如：殺菌不完全的低酸性罐頭食品（pH>4.6之鐵罐、玻璃罐、殺菌軟袋等密封低酸性食品），及貯藏不當的香腸、臘肉等醃製食品，還有俗稱「肉酉圭（讀音：肉給）」的鄉土鹽漬食品，因其多非工廠製作，未經專業殺菌，又不需加熱即可供食，稍有疏忽，即易造成中毒。因肉毒桿菌毒素於加熱100℃10分鐘可破壞，為求安全，應高溫煮沸後再食用。 　　 臺中縣衛生局為確保民眾食用「真空包裝即食食品」安全，已全面進行縣內「真空包裝即食食品」製造業及販售業稽查，目前已完成14家次製造工廠及167件販售業稽查，查核結果均符合規定，並將賡續辦理： 　　 (一)加強稽核真空包裝即食食品製造商落實食品標示，如需『冷藏』之產品應於包裝上確實標明。 　　 (二)輔導超市、超商及傳統市場等販賣商，對於標有『應冷藏』之真空包裝即食食品務必冷藏販售，否則將依違反食品良好衛生規範處辦，且於6月15日辦理「真空包裝黃豆即食食品安全管理業者講習會」計44家業者參與講習。 　　 肉毒桿菌雖可怕，但只要謹守下列預防原則，就可有效防止中毒事件： 　　 1.勿購買或食用來路不明的罐頭，或真空包裝常溫儲存的非乾燥食品。 　　 2.選購罐頭食品須注意有效期限與外觀的完整性。 　　 3.家庭於醃製或保存食品時，應將食物煮沸且須攪拌、控制pH值在4.5以下。 　　 4.購買真空包裝的煙燻或醃製等即食食品，應儘速冷藏保存，加熱後再食...

Diabetic macular edema: Swelling of the retina in diabetes mellitus due to leaking of fluid from blood vessels within the macula. The macula is the central portion of the retina, a small area rich in cones, the specialized nerve endings that detect color and upon which daytime vision depends. As macular edema develops, blurring occurs in the middle or just to the side of the central visual field. Visual loss from diabetic macular edema can progress over a period of months and make it impossible to focus clearly. Macular edema in common in diabetes. The lifetime risk for diabetics to develop macular edema is about 10%. The condition is closely associated with the degree of diabetic retinopathy (retinal disease). Hypertension (high blood pressure) and fluid retention also increase the hydrostatic pressure within capillaries which drives fluid from within the vessels into the retina. Use of the argon laser to close filtering bleb leaks Received: 13 August 1991 Accepted: 3 March 1992 Abstract We attempted to seal 15 consecutive filtering bleb leaks in 11 patients using argon laser energy. Eight leaks were at the conjunctival-corneal interface associated with a fornix-based flap, 4 leaks resulted from suture scissoring of the conjunctiva, and 3 leaks were button-holes in the conjunctiva. Thirteen leaks occurred within 2 weeks after filtering surgery, while 2 were of late onset. Argon laser parameters used were a 500-m spot size, 0.1-s time interval, and between 500 and 1800 mW of power. Thirteen leaks were sealed after one or two laser sessions, while 2 leaks required other treatment modalities. Complications included conjunctival fenestration and transient corneal stromal opacities. Abstract Hyphema resulting from a laser iridotomy (iridectomy) occurred in a patient with chronic subclinical angle closure glaucoma. This complication has been described previously in patients with rubeosis iridis and uveitis and in patients receiving anticoagulant drugs, but to our knowledge, this is the first report of a hyphema resulting from a laser iridotomy in a patient having no known predisposing pathological condition, either ocular of systemic, While no complications of hyphema were encountered in this patient, we recommend that this rare complication be kept in mind whenever a laser iridotomy is contemplated. 筆記： YAG PI要小心造成hyphema，施打前須先問drug history, 以及注意是否有rubeosis
